March 2019 nilimwandikia barua ya wazi (kupitia gazeti la Mwananchi) Raisi wa Tanzania wakati huo Hayati John Pombe Magufuri kumshauri kuichukua agenda ya huduma za afya kwa wote na kuwahimiza wasaidizi wake kuisimamia. Bahati mbaya halikupata
kipaumbele.
Raisi wa sasa ameelekeza wizara husika kuhakikisha linafanyika. Nimelazimika leo kuandika Makala hii kufufua agenda hii na kutoa mapendekezo ya kitaalamu na ya namna ya kuendelea.
Septemba mwaka 2015 Umoja wa mataifa katika mji mkuu wa Marekani New York ulipitisha azimio la maendeleo endelevu la kiafya (SDG 3) na lengo la nane (SDG 3.8) lilikuwa kufikia afya kwa wote ifikapo mwaka 2030. Dhana ya upatikanaji wa huduma ya afya kwa wote umejengwa kwenye msingi wa usawa na haki za binadamu (Kifungu cha 25), hivyo utekelezaji huo utaharakisha kufikia malengo mengine pia.
Septemba 23rd mwaka 2019, umoja wa mataifa ulifanya kikao rasmi kujadili maendeleo na uwekezaji katika afya kwa wote, nchi nyingi zilionekana kusuasua, ikiwemo Tanzania. Shirika la afya duniani liliripoti kuwa angalau mtu mmoja kati ya kumi hapa Tanzania hutumia karibu asilimia kumi ya kipato chake kwa mwaka kwa ajili ya huduma za afya. Ushahidi wa kitaalamu kutoka chuo kikuu cha Imperial College cha London unaonyesha kuwa kuwekeza kwenye afya kwa wote huboresha afya ya jamii na matokeo makubwa yakionekana kati ya watu masikini Zaidi, vifo vya Watoto wachanga vikipungua na wastani wa umri ukiongezeka. Serikali ya Tanzania kupitia Waziri wa Afya kwenye hotuba yake ya mwaka 2018/19 kifungu namba 118 ilisema serikali inalenga kufikia mwaka 2020 Watanzania asilimia 70 wawe na bima ya afya. Mpaka mwaka jana ni asilimia 34 tu walikuwa na bima ya afya. Ili kufikia lengo hilo, serikali ilitakiwa kupitisha mkakati wa kifedha wa kugharimia huduma za afya (Health Financing Strategy), kuwa na mfumo wa kutoa ruzuku ya bima kwa watu wa hali ya chini (subsidized insurance), Kuwa na sera na sheria jumuishi (Social participation). Kwa bahati mbaya, haya hayakufanyika, hivyo kufanya uwezekano wa lengo hilo kuwa mdogo. Kwa vile Raisi ameelekeza mchakato huu uanze tena, hayo ni mambo muhimu kuzingatia. Hatua ya Raisi kuelekeza bima ya afya kwa wote inamaanisha yafuatayo: (a) Kutangaza kuwa kusiwepo na Mtanzania anayekosa huduma za afya kwa kutokuwa na hela, (b) Kupitisha mpango mahususi wa mfumo utakaotumika kuhakikisha watu wa hali ya chini wanakuwa na bima ya afya, (c) Kupitisha na kutekeleza mkakati wa kugharimia huduma za afya (National Health Financing Strategy) na (d) Kuweka malengo ya kuongeza matumizi kwenye sekta ya afya kufikia asilimia 4-5 ya pato la taifa kutoka kwenye 1.7% ya mwaka 2016 kwa mujibu wa Shirika la Afya Duniani na (e) Kujenga na kuhakikisha zahanati zinazotoa huduma zinaongezeka kutoka 43 ya sasa kwenye vijiji mpaka angalau 75%. Ikiwa Raisi Samia akisimamia hili kwa ufanisi, atakumbukwa na watanzania wengi na kupata umaarufu wa kisiasa kama Tommy Douglas waziri mkuu wa Canada ambaye aliitwa Shujaa wa Mageuzi ya Afya kwa Wote mwaka 2004. Raisi wa China Hu Jintao mwaka 2006 alibeba agenda ya afya kwa wote akafikia 95 ya wachina wote na hivyo wananchi kuwekeza kwenye maendeleo kitu ambacho Pamoja na mambo mengine kimeifanya China kuwa nchi ya pili kwa uchumi imara duniani.
